What accounts for this diversity? How have some low- and middle-income countries successfully maintained or even expanded health coverage while others have not? How are the benefits of expanded health coverage distributed? To explore these questions in context, this volume presents eight selected countries in various stages of universalizing their health systems, most of which are emerging economies.

3 It is almost impossible to discuss social policy without reference to the WRA, yet, as was established in a literature review, it is time to abandon this approach despite (or even because of ) what it has offered. Indeed, it might be argued that the more we have learned about regimes, the more we have found them to be deficient. What the approach has demonstrated, to some extent by neglect and omissions as well as by positive contributions, is that social policy theory must accommodate a variety of structural determinants, and how they interact across agencies, processes, relations and institutions to give rise to a diversity of shifting outcomes within and across countries and sectors.
